PLEASE READ THE FOLLOWING CONDITIONS BEFORE SIGNING UP:
1. We do NOT treat chronic pain or prescribe controlled substances. By policy, we refuse ALL requests for chronic pain medicine (like oxycodone, hydrocodone, morphine, etc), benzodiazepine medicines (like Xanax, Klonopin, Valium), and other controlled substances. If you currently require these medicines you may still sign up. We will care for your other issues. You should continue to work with your previous doctor (who knows you and has your records) until we can arrange for you to be seen by a specialty provider. Unfortunately, we cannot make exceptions to this rule. We appreciate your understanding.
2. Your first month will be billed to your credit/debit card for any remaining days this month. Subsequent months will be billed on the first of the month at the standard rate (as discussed in PRICING).
3. We recommend using a computer with plenty of time. This will for the basis of your medical chart. Please be thorough. This will eliminate the need for paperwork at your visit. Your personal information is fully protected (using encyprtion/SSL). YOUR PRIVACY IS ALWAYS TOP PRIORITY.
4. You under no obligation or contract. Both you (or we) may end our relationship at anytime, for any reason.
